I won't claim to be *good* at writing, but I have done a reasonable amount to support my gaming habits. I'm not a total failure with graphics work, but my husband does a good job with his photoshopping when I need it.

Yes, and I'll admit I find it a touch daunting as my Real Life can be a pickle at times. Family/home life, three kids, four depending on how the husband is acting <ahem> plus work. This is how I de-stress, and I am cautious about about making too many IC/OOC commitments in a game, because I hate having to apologize or reschedule a million things simply because the kids are road-testing the latest flu from daycare, etc. Regardless, I'm keen to see how this could work.
15.) What is the best way to contact you?
Honestly, I check my STO mail account regularly, and Forum accounts as well. For now, I recommend STO.
Part B: Role Playing Experience Questions
1.) What does the Term "IC-OOC Separation” mean to you?
This is about keeping your character's reality and your own reality separate. This is essential for impactful story telling. Within the span of an RP'ed story, bad things may happen to your character. Understanding this allows one to fully contribute to a story, and to not become resentful, or to perpetuate unneeded drama, such as when one might assume that the negative things happening to your character are representative of negativity being directed at you. (Sorry if I get wordy!)
2.) What is the difference between the term Player and Character?
Quite similar to the difference between Actor and Character, though it might be argued that in RP, the Player often has a lot more direct control over the character they are portraying.
3.) What is Meta-gaming?
Using knowledge or information your character would not have available to them in an IC context within the context of an RP scenario. Quick example - a GM has an NPC from a Star Trek race with a rare aversion to sound waves at a specific frequency. I as the player know this because I saw the same episode of the Animated Series. My character has never seen this species before, and has done absolutely no work or investigation of her own to determine this weakness. Yet, when I'm asked what my action is... I immediately tune my tricorder and try to render the NPC unconscious with a sonic pulse. That might be an example of poor form, but another way in which Meta-gaming is bad is when one uses knowledge that only another character has possession of ICly. Not only would I be using information my character honestly has no way of knowing, but then I'd be guilty of possibly taking an RP opportunity away from another player. Summary: Meta-gaming = immersion breaking lack of barriers between IC and OOC knowledge.
4.) What are Action Brackets? (:: :: | - - |* *| )
Syntax used in text-based gaming systems to denote a physical action one's character is taking. Other options may include the use of the /em command (E.g. "/em scans for signs of triolic energy" would tell spatial that "Shallya scans for signs of triolic energy".)
OR - is it what Scott put in his application? I can understand if a GM is using this syntax to direct traffic and attention during an RP session.

Since closed beta... so... wow... about a year?

Since the mid-80's when my brother and his friends got me to play in their Robotech RPG.
7.) What was your last role playing experience?
Since the summer, I've been RPing in STO with some friends who migrated from Star Wars Galaxies. It was a good run, but as many others have found, hitting VA seems to suggest that STO is 'over'. The addition of Foundry to STO is really going to help retention, but that isn't what you were asking about =)
8.) What are your expectations of this fleet?
I am looking for a good RP home for at least the next calendar year if not longer. I've invested in STO, and have been a lifetime Star Trek fan. So far, I've been involved in two, really good, yet short-lived RP fleets. My primary expectation is one of longevity, but that is still secondary to having opportunities for quality, Trek RP with like minded folks.
9.) What will you bring to this fleet?
I'm an avid roleplayer, I love what I see on Tribble with Foundry (If you dabble on Tribble, try 'Bad Times in the Borderlands' for a perspective on my story telling style.) Basically, I'm committed to this game, and would like to share my energy for it with this fleet.
10.) How do you feel about PVP?
I can take it or leave it. Some of the epic PvP battles I fondly remember in SWG were part of what kept me coming back to that game (I hope that doesn't disqualify me instantly!). However, I have to admit I *hate* what passes for endgame raids, special task forces, and similar 'high-level' content. I seriously do not enjoy trying to fine-tune my build to survive some brutalization of my character just to earn something coded purple. Ick! I also wrinkle my nose up at the current PvP state of affairs in STO (Apologies for my tangent!)... these 'arenas' are sad. I hope that in 2011, there's some kind of meaningful PvP between factions that might be more enjoyable than what is available today.
That being said, I've no issues trying to join fleet mates for PvP. I don't use voice chat, so that's sometimes a barrier, so I tend towards healer/support ships, versus combat point.
11.) What difficulty do you typically play Star Trek Online on?
Normal or advanced. I'll play normal for STO-generated content. I find that their approach to scaling difficulty is silly. If I'm flying/fighting with people I know well, Advanced is fine. I love the damage system you get when playing at the higher challenge levels.
12.) Do you understand that all role playing is conducted locally?
I believe so. This refers to local/spatial chat as being an RP-live channel.
13.) Do you know how to roll dice on Star Trek Online? (If so what is the command?)

Shallya Chase is a skilled physician who seeks to explore the galaxy. She was raised by Bajoran medical and relief workers, and grew up and into a tense lifestyle supporting and relocating the numerous refugee colonies that sprang up as the result of the Dominion War. Uncomfortable with a lifeplanet sidee, she applied to and was accepted at Starfleet Academy. Despite several misadventures as a cadet, she graduated, and completed her medical training. The realities of war have seen her assigned tofront linee surgical units, and starbase-hosted medical facilities. Her adventurous nature often leads her into challenging situations where her obligations as a healer come into conflict with her fiery, Bajoran temper.
9.) What kind of commission does your character have? (Enlisted, Petty Officer, or Officer)
Officer.
10.) Desired ship type of ship you'll be wish to be assigned too? (Tactical, Science, or Cruiser).
Cruisers or Science ships are preferred, but escorts can be fun too.

During her childhood, Chase was exposed to three consistent themes. The first was medicine. Her parents were Bajoran doctors who answered the will of the Prophets through their unwaveringcommitmentt to the many relief efforts that were needed following the end of the Dominion War. The second theme was that ofreligiouss service. Chase was brought up to believe that the guidance of the Prophets saved Bajor during the war, and that service to those in need was a high calling. The third theme was that of inadequacy. Her parents had fought in the war, the greatest conflict seen in the Alpha Quadrant, and no matter how hard she tried to help their medical missions, she knew she could never live up to the legacy of her parents' generation.
Having spent so much of her youth on medical ships, Chase found adjusting to life on Bajor difficult in her late teens. Her parents had settled down to practice their craftplanet sidee, but in her heart, something still called Chase into space. She followed her instincts into Starfleet Academy, where she went into overdrive. She advanced the basic medical training she had into a full fledged career, and the dream of a posting to an exploration ship drove her to success.
The unfortunateeruptionn of conflict for the Federation posed several challenges for Chase. The posting to a deep space ship did come upon graduation as she was assigned to a front line medical unit. In addition, the shine wore off the previously glittering enthusiasm she held for her future. The realities of treating war wounded on a daily basis caused her many moments of self doubt. The intensity of her posting might have burned her out, if not for a reserve of Bajoranstubbornnesss that managed to see her through the worst days of her tour of duty.
Her postings since have been to Starbase hosted medical units, which although interesting had a far lower adrenaline factor than she was used to. An opportunity to transfer to a starship eventually materialized and she applied, hoping to find that place where she could make a difference, like her parents once did.
12.) Please describe what Merit System is.
My take on it is this - a peer reviewed, and IC system of rewarding IC actions. I'll resist my temptation to steal Scott's answer to this question... and just admit that I also believe that ICA=ICC.
13.) Has this character been enrolled in a fleet prior?
This character has not been in any prior fleet.
14.) What are your aspirations for this character? Casual RP (I am just here to RP) or Competitive (I want to be an Admiral some day!)
I'd hesitate to look too far into the future. I do want to advance my character, but I honestly gravitate towards medics and scientists because I'm concerned largely with RP over command or positions where rank is truly significant.
15.) What are strengths, weakness, and flaws in this character’s personality?
Strengths - passion for medicine, resourcefulness, determination
Weaknesses - Thrill seeker, succumbs to self doubt, has a short temper

2401 - Competes in Mayweather Shuttle Derby. Equipment malfunction results in collision with Cadet Michael Scott. The two competitors spend 2 weeks an a frigid, L class planet in the Draylax system. Their instructors commend them for excellent use of their survival skills.
2402 - Graduates Starfleet Academy. Posted to Starfleet Medical to complete PhD and medical school.
2406 - Dr. Shallya assigned to front line mobile hospital unit 47.
2408 - Assigned 3 month medical stress leave. Performance otherwise spotless.
2409 - Detained by authorities along with numerous citizens involved in an anti-war protest while on leave on Terra. Released when it was realized she was trying to assist those injured in the protest.

2412 - Commendation for bravery after leading a relief convoy of runabouts loaded with critical medical supplies past an enemy blockade tobesiegedd colonists.
