To: Crew of the USS Sentinel From: USS Sentinel Medical Staff Subject: Annual Physicals Stardate: 92363.00
It is that time again, for required medical exams. Members of the crew should schedule an appointment for their physicals as soon as possible.
OOC: I'm throwing this out there for anyone interested. This can be done in game, on the forums, or whatever else you might have in mind. Or even not at all if no one is interested.
Although it would be a good opportunity to put some IC information regarding any special physical (or maybe even psychological) attributes your character might possess.

OOC: The following is a brief medical questionnaire, you know the sort you fill out at the doctors office. I would love it if Sentinel crew could fill this out, for the record. I realize some of this information is included in some people bios, but please fill out at least the questions that are not as well as any questions to which the answers have changed. Thank you. You may PM the results to me or post them on this thread, ICly please since this an IC thread. Also if your basic physical data is not included in your bio please include it here (you know height, weight, eyes, hair, embarrassing tattoos , etc.Medical Questions

To: Doctor Laurel Orkney-Winterhagen From: Doctor Billie Jo Spencer
Dear Laurel,
I gave Werner the "talk" about his diet and the smoking habit. With any luck that will convince him it is an important health concern, and not just you nagging.
I hope everything is going well for you. Hopefully I can get by some time and visit. Give my love to Zoe and K'Rika.
Best Wishes Billie Jo

Medical File: Ensign Adryan, Sebetharen D.
--------------------------------------------------------------------------------------------------------------------------------------
Name: Sebetharen Dimol Adryan Race: Trill/Romulan Telepath: No Gender: Male Age: [in Earth years] 34 Size: [in metric units] 1.74m Weight: [in metric units] 78.47kg
--------------------------------------------------------------------------------------------------------------------------------------
Branch: Operations Rank: Ensign Duty Clearance: Cleared
--------------------------------------------------------------------------------------------------------------------------------------
Mental State: Typically cold and detached personality despite his intellect. Exhibits extreme aggressiveness when feeling threatened or challenged. Has an issue with empathizing with others, choosing more definitive results and ignoring ones of a more emotional nature. Territorial, displays extreme protective and self preservative instincts. The latter manifests itself in odd ways: Seems to have an enhanced instinct which, in combat, he seems to solely rely on. At times the decisions made in combat may seem insane to a spectator but as of current have been successful. "Does not play well with others" is an understatement. Extremely standoffish and crude speaking. Has had altercations with other officers. He seems to make exceptions to those he respects and he while has not admitted it he seems to have a soft spot for his subordinates.
Note of concern: He refuses full dermal regeneration, as a result an otherwise attractive countenance and physique is riddled with scars. These mars are easily repaired, but he seems to have no desire to do so.
--------------------------------------------------------------------------------------------------------------------------------------
Physical State: BMI 25.9% BFP 8.9% Entire body marred with scars. Face: Three jagged scars on the left cheek, one on the right, back of the head has several small scars, left side behind ear is a laceration scar. The scars on his body are too numerous to list, but consist of everything from perforations to burns.
Fitness level is well above minimum requirements. Sub Note: Body ages slower than the typical Trill, likely due to the longevity effects of Romulan genetics. --------------------------------------------------------------------------------------------------------------------------------------

Medical File: Lieutenant Commander Decia Valerina Maximina

Name: Decia Valerina Maximina Race: Magna Roman Telepath: No Gender: Female Age: [in Earth years] 28 Size: [in metric units] 1.70m Weight: [in metric units] 54.43kg

Branch: Operations Rank: Lieutenant Commander Duty Clearance: Cleared
---------------------------------------------------------------------------------------------------------------------------------------------------------------
Mental State: Warm, friendly and approachable when off duty, Precise, and professional about her duties retaining a formal and almost noble bearing. Decia also appears to exhibit an almost natural desire to protect those physically or emotionally unable to protect themselves. She bears a desire to project the perfect image of a solider from her homeworld. By pushing herself to the limit she has attained this image of the perfect legionnaire no doubt instilled within her since birth.
A Natural leader who truly cares for those under command and protection. She does not seem willing to ask anything of any of those she leads into battle she is unwilling to ask first of herself.
Note of concern:
She places such a high value on the lives of others, she would think nothing of sacrificing her life if it would save another life.
---------------------------------------------------------------------------------------------------------------------------------------------------------------
Physical State: BMI 19.4 BFP Subject only bears a few scars. One left side abdomen, One center chest recent injuries sustained in combat. Plasma burn from Herald weapon back below left shoulder blade. Disruptor burn located lower hip left side.

Name: Victor Jackson Springfield Race: Human Telepath: No Gender: Male Age: [in Earth years] 35 Size: [in metric units] Weight: [in metric units]
--------------------------------------------------------------------------------------------------------------------------------------
Branch: Intelligence Rank: Lieutenant Duty Clearance: Cleared for Duty
--------------------------------------------------------------------------------------------------------------------------------------
Mental State:
Mr. Springfield exhibits a generally friendly demeanour with the occasional bout of paranoia.
[DECLASSIFIED FOR MEDICAL PURPOSES] Mr. Springfield's mental state may be affected by accidental exposure to an anomalous four-dimensional object. The effects of this object are unknown, but others who were exposed to that same object suffered incurable insanity.
[DECLASSIFIED FOR MEDICAL PURPOSES] Mr. Springfield's mental state may be affected by a series of remote viewing experiments called the Watchtower Project. During these experiments, his consciousness was detached from his body on twenty separate occasions. The long-term effects of this experiment are unknown.
--------------------------------------------------------------------------------------------------------------------------------------
Physical State:
"Mr. Springfield has no known health issues at this time, aside from a decreased lung capacity of about 15% due to repeated inhilation of smoke derived from the combustion of tobacco. It should be noted that this antiquated hobby -- which is almost entirely forgotten in modern society -- places him at a greater risk of developing cancer of the mouth, lung, and throat. His response when confronted with this reality is, and I quote, 'You can cure cancer, so it's no big deal.' No further comment." -- Dr. Valen Araan, Field Agent Clinical Supervisor, Project Avalon.

----------------------------------------------------------------------------------------------- Medical File: Lieutenant Stingray, Val
-----------------------------------------------------------------------------------------------
Name: Val Stingray
Race: Betazoid
Telepath: Yes
Gender: Female
Age: [in Earth years] 30
Size: [in metric units] 1.62
Weight: [in metric units] 59 kg.
----------------------------------------------------------------------------------------------- Branch: Science
Rank: Leiutenant
Duty Clearance: Cleared
-----------------------------------------------------------------------------------------------
Mental Status: [Current(Corrective Commentary by Commander Storm)] Manic, but controllable with meditation and use of the Betazed Medical Service Hologram (BMSH; designate: Elusive). It is easy for the subject to lapse into prolonging stages of rapt focus, continuing along a particular thought path till interrupted. It has proven to be a useful quality, but, the subjects attention can be caught on something purely trivial, and can be easily distracted. This has not proven a problem in a work environment as long as the subject is working around others who can direct her creative talents (bullshit, she functions just fine in a work environment. No one needs to hold her damn hand.)
She is highly intelligent with a keen analytical mind. Before being Assimilated(let's hold that against her) she put out several papers on physics and Astrophysics of merit.
The subject is well received by others and goes out of her way to interact with others. Her extroverted nature makes her suitable to return to a starship post should the opportunity arise(ASAP; we're just holding her back).
Subject previous medical records have been added from the Starfleet Medical Hospital, Betazed Division:
This a summary of progress one Val Stingray after being rescued from the Collective. After six months Val has made incredible progress in retaining her individual identity. I am happy to say that the careful young woman has returned, though the scars of the incident will longer. Two years is a short time compared to some who have been pulled out of the Collective, but feelings will continue to linger. No one can be violated like that and just get over it. I am happy we were able to break her out of the fugue state. The incident with the [Classfied: Breach of Patient Confidentiality. Commander Storm]. The rate of healing and reassociation, I believe she will be able to reintegrate back into society with no fear of the voices in her head being the Collective. It is a shame that a talented telepath was made to fear her own gifts.
She asked me if it was possible for her to go back to Starfleet. I asked her why she would want to go back. She stated that her going back mean they lose. That the Collective could not drive her from space and away from her passions. She still wants to go out and explore. I have to give her credit, she has a fire in her to persevere. I can respect that.
Rumors of subjects rehabilitation seem to follow after incidents prior to her coming out of a fugue state. There have been misleading or salacious rumors of Stingray[Classified: Breach of Patient Confidentiality. Commander Storm. Insert: I'm going to beat you to death with a barbell, you damn Vulcan.]
After two years of physical and mental rehabilitation, Val Stingray is allowed to return to Starfleet, retaining her former rank of Lieutenant, junior grade (about damn time).
-----------------------------------------------------------------------------------------------
Physical state: BMI 25.2
-----------------------------------------------------------------------------------------------Due to abduction by the Borg and subsequent experimentation, Val Stingray has received extensive bionic and cybernetic reconstruction. Due to the nature of some of the Borg implants, replicated organs were deemed dangerous to Stingray's biological stability.
Fitness level at adequate requirements.
List of Bionic/Cybernetic augmentation. Patient has indicated that she desires normal physical parameters of a Betazoid woman of her height and age. She has declined further study of her neural implants at this time.
Borg Implant: Neural pathway reinforcement. The implant is visibly displayed on subjects skin. In low light settings, the implant is appeared visible to the naked eye. Subject has indicated that she feels uncomfortable displaying the implant and fully covers herself. The implant seems to have allowed for subjects nervous system to conduct energy at a high rate through the entire body. The neural pathway of the brain has been doubly reinforced and allows a faster transference of data to be processed.
Speculation: It would seem that the implant was meant to allow subject to be plugged into a larger system where she would be allowed to conduct extreme amounts of energy and data.
Forearm and hand: Bionic replacements. At the stomp of each arm is a Borg implant that allows the replacement systems to interact with the rest of the body. the Borg implants remain due to the fact that removing them would result in the removal of the upper arms of the subject.
Lower legs and feet: Bionic replacements. At the stomp of each arm is a Borg implant that allows the replacement systems to interact with the rest of the body. the Borg implants remain due to the fact that removing them would result in the removal of the upper legs of the subject.
Speculation: The Borg implants seem to be key sources of the Neural pathway reinforcement to receive direct contact with an energy source. There are no known records that we know of about the Borg where a drone would need such augmentation.
Note: The Borg's experimentation leads to the subject internal organs to be removed and replaced with cybernetic constructs. Starfleet Medical has replaced the constructs with their own and attuned them to her biological system. Subject has requested no further argumentation via the cybernetic systems. She does not wish to experiment with further advancing her physiological capacity.
Cranium Implants [Classified: Ultrasecret. Taskforce Omega: 505-Omega]
Despite best efforts, the subject still bears bionic augmentation scars and Borg implants that could not be removed without threat to the subject health.
